Transferring Care

We are honored you’re considering Physicians and Midwives for your care.

Whether you are new to the area or transferring care from another local practice, we want to make the process as smooth as possible. Because of our unique collaborative care model, we are able to accept the vast majority of transfer requests we receive. If you are considering transferring pregnancy care, please reach out to us as early in your pregnancy as possible.

Transfer Obstetrics (Pregnant) Patients

Less than 24 weeks gestation:

  1. Schedule your first visit: Please call us at 703-370-4300 to schedule your first visit and let us know you are less than 24 weeks pregnant and seeking to transfer your pregnancy care to Physicians and Midwives.
  2. Obtain your medical records: Please obtain a copy of your records from your former obstetrical practice prior to your first appointment with Physicians and Midwives. Please have them fax your records to us (703) 370-0044, mailed to the Alexandria office, or bring a paper copy with you to your first appointment.
  3. Prior to arriving for your first visit: Please bring your records with you or verify we have received records prior to arriving to your first visit by calling 703-370-4300. If we do not have your records prior or on the day of your appointment, we will not be able to see you for your scheduled visit and will need to reschedule your appointment.

24 week gestation and above:

  1. Contact Physicians and Midwives: Please call us at 703-370-4300 to let us know you are greater than 24 weeks pregnant and interested in transferring your pregnancy care to Physicians and Midwives. Our staff can review this process with you and answer any questions you may have.
  2. Obtain your medical records: Please obtain a copy of your records from your former obstetrical practice prior to scheduling your first appointment with Physicians and Midwives. Please have your records mailed to our Alexandria Office, faxed to us at (703) 370-0044, or feel free to drop them off in person. Records should contain lab-work, ultrasound reports, OB flowsheet, and all appointment notes. If your records are mailed or faxed, please call us 703-370-4300 to let us know they have been sent and to verify receipt.
  3. Medical Record Review: A physician will review your medical records and determine if the practice is a good fit to meet your clinical needs based on your current pregnancy and medical history. We also review whether the practice can accommodate additional transfer patients based on volume of current patients expected to deliver each month. The records review process is in place to ensure the practice can meet your needs before you transfer care. This review process typically takes 3 business days, but it could be up to 10 business days from when your records are received depending on the volume of records. We will not be able to see you for any appointments until your medical records has been reviewed and you have been formally accepted into the practice.
  4. Schedule your first visit: After your records have been reviewed and you have been formally accepted to the practice, we will contact you and assist you in scheduling your first appointment.

Transfer Gynecology (Non-Pregnant) Patients

If you have medical records or recent lab results, please feel free to bring with you to your appointment so your provider can review, and we can scan into your chart. If you do not have your historic records, your provider may ask you complete a medical release form so that we may request any applicable records from outside practices and healthcare systems.

If your visit is following up from a recent hospitalization, please bring any hospital records you have so your provider can review with you at your visit and give you the best care.

Sending Medical Records to

If you have sent or asked for records to be sent to us, please confirm your records have been received prior to your appointment by calling the office at (703) 370-4300. Please keep in mind that we cannot accept records via disk, CD-ROM, DVD, or USB memory drive for IT security reasons.
